Medical/Emergency Action Plans

Any medical condition that requires management at school must have an appropriate Care Plan completed by the physician and parent in addition to the Medication Authorization forms. These must be on file on the first day of school.

Any medication required at school requires a Medication Authorization form completed by both physician and parent. This applies to over-the-counter medications as well. Medication is to be in a properly labeled container from the pharmacy or in a sealed over-the-counter package.

As a reminder, parents must check in at the Welcome Center before entering the building.
