Health Examination/*current* within the last calendar year*
(A Sports physical does not serve as a School physical,
though a School physical may serve as a Sports physical)
Required Immunizations are:
3 doses of DTP/Dtap (with the last dose qualifying as a booster and received on or after the 4th birthday)
3 doses of Hep B
3 doses of or more of Polio (with the last dose qualifying as a booster and received on or after the 4th birthday)
2 doses of MMR (The first dose must be on or after the 1st birthday)
2 doses of Varicella (The first dose must be on or after the 1st birthday)
1 dose of Tdap
1 Meningococcal 1 dose for 9th, 10th and 11th grades
2 doses for 12th grade (The second dose must be on or after the 16th birthday)
Please make sure all immunizations are up to date.
Any Medical Condition that requires management at school, must have an appropriate Care Plan completed by Physician and Parent in addition to the Medication Authorization forms. These must be on file the first day of school.
Any medication required at school requires a Medication Authorization form completed by both Physician and Parent. This applies to over the counter medications as well. Medication is to be in a properly labeled container from the Pharmacy or in a sealed over the counter package.
As a reminder, Parents must check in at the Welcome Center before entering the building.

As of the Fall of 2015, all students entering the twelfth grade will be required to show proof of a recent meningococcal conjugate vaccination (MCV). All twelfth grade students will need to show proof of receiving two doses unless the first dose was administered after 16 years of age.
