Incoming Senior Health Requirements

As of the Fall of 2015, all students entering the twelfth grade will be required to show proof of a recent meningococcal conjugate vaccination (MCV). All twelfth grade students will need to show proof of receiving two doses unless the first dose was administered after 16 years of age. All seniors must have the 2nd vaccination BEFORE the first day of school. If no proof of vaccination has been submitted, the student will not be permitted to attend until proof is submitted to the nurse. If you are unable to schedule an appointment before this date, an appointment needs to be made and in order to not be excluded we will need a doctor's note stating the appointment date and time. 
 
 
Acceptable documentation of meningococcal conjugate vaccination includes:
  1. Letter signed by your health care provider indicating the dates of vaccination OR
  2. Printout from your health care provider's electronic medical record indicating the dates of vaccination (this page needs to be signed and dated by the health care provider)
  3. Current Certificate of Child Health Examination signed by your health care provider indicating the dates of vaccination

If you are unable to submit health documents electronically, please drop them off at Riverside Brookfield High School.
 
Please ensure you keep copies of all health documents for your personal records.
