Transfer Students
In-State Transfer Students (Within Illinois):
If your child is transferring from another Illinois school, you must provide:
- Physical Examination & Immunization Records
- Submit the most recent physical exam and up-to-date immunization records
- The physical must be documented on the Illinois Certificate of Child Health Examination Form
- Physical must be turned in to the health office in order to receive schedule/enroll
- Dental Examination
- Applicable for students entering 9th grade
- Must be turned in on a Proof of Dental Examination Form
- The dental exam must have been completed within 18 months prior to May 15 of the current school year
Out-of-State or Out-of-Country Transfer Students:
Students transferring from outside Illinois must provide:
- Physical Examination & Immunization Records
- A physical exam completed within one year prior to enrollment, documented on the Illinois Certificate of Child Health Examination Form./ Spanish version
- Up-to-date immunization records as per Illinois requirements
- If immunizations are not up-to-date a catch up schedule needs to be in place in order to start
- These documents must be submitted within 30 days of enrollment
- Eye Examination
- Required for all students enrolling in an Illinois school for the first time
- The exam must be conducted by a licensed optometrist or physician and completed within one year prior to enrollment
- Submit the Illinois Eye Examination Report form by September 15 of the current school year or by 30 days after enrollment
- Dental Examination
- Must have been completed within 18 months prior to May 15 of the current school year
Submission Deadlines:
- Physical & Immunization Records: Due by September 15th of the school year
- Eye Examination: Due by September 15th of the school year
- Dental Examination: Due by May 15th of the school year
- Out-of-State Transfers: All required documents must be submitted within 30 days of enrollment
Note: Failure to submit the required health documents by the specified deadlines may result in the student being excluded from school until compliance is met
