Greetings Parents/Guardians, Students, and Staff,
Parents/guardians, students, and staff/teachers throughout the state of Illinois will have an opportunity to participate in the annual, statewide 2023-2024 Illinois 5Essentials Survey starting on Tuesday, January 30, 2024. This survey is designed to give a detailed picture of the climate and culture of your student’s school, Riverside Brookfield High School (RB), as it relates to the critical elements of school effectiveness. Survey results will be used to help the administration identify strengths and areas of growth to make RB even better.
The Illinois 5Essentials Survey identifies five indicators that can lead to improved student outcomes: effective leaders, collaborative teachers, involved families, supportive environments, and ambitious instruction. Research has shown that schools demonstrating strength in these indicators are significantly more likely to improve student academic performance.
The Illinois 5Essentials survey is administered annually by the University of Chicago and facilitated locally by schools via an online portal. All survey responses will be kept confidential and will only be reported as part of the comprehensive school report, not individually. The participation rate goal is 95% for all stakeholders.

To take the brief survey, please follow these steps:
Parents/Guardians
- Visit https://survey.5-essentials.org/illinois/?target_name=parent
- Select language preference
- Type your email address
- Type and Select “Cook” as “County”
- Type “Riverside Brookfield Twp HS” into the “School” field, select the school, then log in.
- Answer all applicable questions
- Click submit survey
- Approximately 10 minutes to complete
*Students
- Visit https://survey.5-essentials.org/illinois/?target_name=student
- Select language preference
- Enter state student ID (SIS ID#) and birthdate.
- Answer all applicable questions
- Click submit survey
- Approximately 20 minutes to complete
*To be completed on Tuesday, January 30 during extended 2nd period.
Staff/Teachers
- Visit https://survey.5-essentials.org/illinois/survey/teacher/login/
- Enter email and passcode (emailed by UofC)
- Enter requested demographic information
- Advance to survey questions.
- Answer all applicable questions.
- Click submit survey.
- Approximately 10 minutes to complete
